For your convenience, we have provided forms for you to fill out in advance of your appointment. We ask that you complete the patient health history, HIPAA privacy notice and Financial Agreement. See Patient Forms.
Your scheduled appointment time has been reserved specifically for you. If you are unable to keep an appointment please call the office as soon as possible to cancel. Our office staff will be happy to reschedule a new appointment for your child. We ask that you kindly provide us with at least 24 hours notice so that we can offer this appointment to another patient.

We accept cash, check, Visa, Mastercard and American Express as well as debit cards with the Visa or, Mastercard emblem. Please call Dr. Atherton’s office for information about discounts for day-of-service cash or check credit card payments.
We work with ALL insurance providers. We are also a preferred provider for Premera Blue Cross Choice Network (including Microsoft), Delta Dental, Cigna DPPO,and DHA which includes Assurant, Aetna, Dearborn, Sun Life, United Concordia, and Unum. Please call the office for more specific information regarding your insurance policy.
For your convenience, we electronically file all claims on your behalf. At your request, we will assist you in estimating your portion of the treatment cost but, we cannot guarantee how your insurance company assigns benefits to each claim. Ultimately, the responsibility for payment of the claim belongs to you. If needed, we will work with you regarding a payment schedule.
